The Senior Vice President for Academic Affairs appoints a fact-finding review panel to conduct the inquiry. Within 15 calendar days after the review panel determines its recommendation, the chair of the panel reports that recommendation to the Senior Vice President for Academic Affairs and sends a copy of the recommendation to the unit dean, the ombudsperson, the complainant, and the accused.

If not satisfied with the result of the inquiry, the complainant makes a formal allegation within five calendar days. The Senior Vice President for Academic Affairs also determines within five calendar days if the report warrants a formal investigation.
