The University may request recertification of medical conditions to support the leave, but not more than every thirty (30) days, unless:
1. 1. Employee requests an extension of leave,
2. 2. The circumstances described by the original certification have changed significantly (such as the duration of the illness, the nature of the illness or complications),
3. 3. The University received information that casts doubt on the continuing validity of the certification, or
4. 4. The employee is unable to return to work after the leave because of the continuation, recurrence, or onset of a serious health condition.
During the leave, the employee must report to the supervisor on a monthly basis, the status of the leave and the employee’s continuing intent to return to work.
