3.3.3.2: Life Insurance, Accidental Death and Dismemberment Insurance

The University provides full-time employees who complete the appropriate application form group term life insurance equal to the employee’s annual salary. The policy becomes effective the first of the month following date of hire. The University also provides accidental dismemberment insurance coverage equal to one-half times the University’s life insurance amount in the case of loss of an eye, hand, or foot due to an accident. In the event of accidental loss of any two or more of the above, the benefit payable is equal to the University’s life insurance amount.
